As your summer plans are wrapping up please keep in mind that it is a great time to make sure the school checklist is caught up.  Below is a list of specific grades and what the State of Iowa requires for students to return to school in the fall.  Look it over, get appointments made and the list checked off.  If you need a form please don’t hesitate to let me know, I’ll be happy to provide you with one.  Completed forms can always be emailed to me or turned in before school starts.

Kindergarten

• Up to date immunizations + kindergarten boosters

• Birth certificate (if not on file)

3rd Grade

• Vision Screening

7th Grade

• TDap

• 1st dose of Meningococcal Vaccine

7th-12th ATHLETICAL

• Sport Physical if playing school sport

• Concussion Form

• Emergency Form

(ALL MUST be turned in before your student will be allowed to even start practice)

9th Grade

• Dental Screening

12th Grade

• Dose of Meningococcal Vaccine

What: Sports Physicals done by Dr. Nate Witchman

Who: All Grade 7-12 student athletes, dance team, cheerleaders, and marching band/flags.  

When: Tuesday, August 9, 2022 From 5:00-8:00 PM 

Where: Riverside Middle/High School (18997 Hwy 59Oakland, IA 51560)

The cost is $25, Dr. Witchman can take cash, check or card.
