Riverside Community School District is offering flu vaccines during our annual Flu Shot Clinic
Date: Friday October 22, 2021
Time:9:00 AM -- 3:00 PM
Where: Oakland Elementary, Carson Elementary, and Oakland Middle School/High School
What to bring with you: Immunization Consent Form (your student can get one at the school they attend) and a paper copy of Health Insurance Card to include the address of the insurance company. Please understand your insurance coverage, you are responsible for any costs not covered.
Please be sure to read the entire Consent Form and fill out the form completely. If the form is returned incomplete, without signature or without a paper copy of insurance card the student/staff member will not be eligible for vaccination that day.
